1. Online Application Form only if you are paying by credit card.
2. Print out the PDF of the Vision Plan Application Form. After completing the application form, please mail to NYSUT Member Benefits Trust, Attention: Voluntary Vision Plan, 800 Troy-Schenectady Road, Latham, NY 12110-2455.
3. Request an application form with Pension Deduction Authorization Card by calling Member Benefits at 800-626-8101.
Upon receipt of your form, your application will be processed and a confirmation of vision plan enrollment will be sent to you in approximately two weeks.
Once you’re confirmed, call an in-network provider, identify yourself with your name or NYSUT ID number (see your NYSUT membership card) and make an appointment. The provider’s office will contact Davis Vision and verify your eligibility for benefits. No claim forms or ID cards are needed.
The NYSUT Member Benefits Trust-endorsed Vision Care Plan is provided by Davis Vision and administered by the NYSUT Member Benefits Trust.
The Davis Vision Voluntary Vision Plan is a NYSUT Member Benefits Trust (Member Benefits)-endorsed program. Member Benefits has an endorsement arrangement of 7.9% of premium. All such payments to Member Benefits are used solely to defray the costs of administering its various programs and, where appropriate, to enhance them.
