ID Watchdog Platinum Coverage includes advanced identity monitoring of a member’s personal, identifiable and financial information. The plan includes:

COST OF PLAN:
The cost of the program is based on the size of the group.
1.
250 Members or Less -- $7.50 per person, per month for individual coverage & $11.75 per month for family coverage.
2.
251 to 1,000 Members -- $7.00 per person, per month for individual coverage & $11.25 per month for family coverage.
3.
1,001 to 5,000 Members -- $6.00 per person, per month for individual coverage & $10.75 per month for family coverage.
4.
More than 5,000 Members -- $5.50 per person, per month for individual coverage & $10.25 per month for family coverage.
In the event that the group provides individual coverage, participants have the option of adding family coverage at the cost of $16.95 per month.
